Mesenchymal stromal cells (MSCs) are delivered directly into the lungs using a bronchoscope (CIBA method), where they migrate through the alveolar space and interact with immune cells—such as alveolar macrophages, M1/M2 macrophages and regulatory T cells. This interaction promotes the release of anti-inflammatory signals (like IL-10) and reduces harmful cytokines (such as TNF-α and IL-6), helping to control inflammation in patients on ECMO.
