White matter lesions modifying endovascular therapy outcomes in large ischemic core stroke: A secondary analysis of the ANGEL-ASPECT trial (IMAGE)

Graphical summary of a secondary analysis of the ANGEL- ASPECT randomized trial (N = 456) evaluating whether baseline white matter lesion (WML) severity on noncontrast CT modifies the clinical benefit of endovascular therapy (EVT) versus medical management (MM) in acute anterior-circulation large-vessel occlusion stroke with a large ischemic core. EVT was associated with a favorable shift in 90-day modified Rankin Scale (mRS) scores and higher rates of functional independence and early neurological improvement among patients with absent-to-moderate WMLs, whereas the estimated benefit appeared less pronounced in those with severe WMLs; however, treatment-by-WML interaction tests were not statistically significant.
