(Left) In patients with focal onset seizures without generalized seizure, excitation is under control to some degree by the inhibition augmented by increased AMPARs in the same region where AMPAR-coupled pathological upregulation of excitation is observed. (Right) In patients with FBTCS (focal to bilateral tonic-clonic seizure), AMPAR-coupled disinhibition is considered to spread in the brain, thus causing a shift from the focal to generalized seizures.
